Robert Osfield 5b75d7f525 From Michael Gronger, addition of ReaderWriter* Registry::getReaderWriterForExtension(const std::string& ext).

Also removed copy constructor and = operator from Ouput as it was produce
spurious warnings under gcc 3.3.
